Please review the Community Event Guidelines at the bottom of this form prior to submitting your application.  Following submission, we will review your request and contact you regarding next steps, including your execution of the Activision Tournament License Agreement or other agreement. All event requests must be approved by Activision in its sole discretion.

Provide as much detail about the event in each field below. All fields should be completed. Enter N/A if not applicable. Incomplete forms may result in processing delays.

Please note that you may be contacted by Activision for a post-event report. All information will be kept confidential and will be used by Activision for internal information gathering/learnings.

ALL EVENT REQUEST FORMS MUST BE SUBMITTED AT LEAST 2 WEEKS PRIOR TO PROPOSED EVENT DATE. ACTIVISION WILL NOT APPROVE ANY EVENT REQUEST THAT HAS NOT BEEN SUBMITTED USING THIS FORM.
